At FACCES, we know true community wellness takes more than just one program or one resource. It takes a team of committed partners working together. One of those partners is the OSU Community Care Coach, a resource that’s been bringing healthcare directly into our neighborhood.
“The OSU Community Care Coach has been instrumental in breaking down barriers to care,” said Lilian Hetson, MD. “For many, transportation or the fear of going into a doctor’s office can be a major hurdle. By showing up directly in the community, we make healthcare accessible and comfortable.”
That accessibility matters most when addressing chronic health conditions like diabetes and high blood pressure—issues that deeply affect families in our community. The OSU Community Care Coach also offers preventative care such as cervical and colon cancer screenings.
“We hear from families all the time that if we weren’t here, they probably wouldn’t get the care they need. Being on-site changes that; it makes care feel possible,” Dr. Hetson added.
Dr. Hetson also emphasized how much the OSU Community Care Coach values FACCES’ holistic focus on wellness.
“As a community partner, we really appreciate how FACCES addresses health from so many angles,” she said. “From food scarcity initiatives to sustainable practices, you’re not just helping with healthcare—you’re creating a healthy community environment.”

This alignment between healthcare and holistic community support creates a lasting impact. When families have access to nutritious meals, education, and supportive spaces, they are better able to manage their health and thrive.
The OSU Community Care Coach is just one of the many Faces of FACCES working to build a stronger, healthier community.
