SELECT A PROGRAM
To get started, input your address to browse programs in your area. Select an early learning program to contact and confirm their willingness to accept your family.
Your Child’s Physical Exam
A physical exam form is required to verify your child’s health and wellness prior to enrollment in early learning.
Click here to download and print the form. It must be completed by your child’s doctor or other health professional. The completed physical exam form should be submitted to the early childhood program.
Other Documents Needed
The program you contact will instruct you on what other documentation is needed to enroll your child. This may include income verification, pay stubs, and proof of address.
